Tender Loving Care Home Hospice specializes in medical hospice services for those families and patients diagnosed with a terminal illness. Affectionally known as TLC, they have a great team of professionals that work as an Interdisciplinary team to accomplish the care appropriate to each patient and their family.

TLC Home Hospice is also State Licensed, Medicare Certified and JCAHO Accredited. This is what makes US different – being prepared to care for those who have entrusted us to care for their loved one.

Moorpark Health Care Center is like a home away from home. Located on a residential street in the City of Moorpark, this medical facility radiates with love, care and compassion. This beautiful 4700 sq. ft. custom home facilitates 10 beds, offering 24 hour skilled nursing care in a home setting. State of the art surveillance equipment is employed 24 hours a day. We give patients and families an alternative to skilled nursing facilities. You and your loved one can enjoy the comforts of home at the Moorpark Health Care Center. Other services we provide are dietitians, physical therapy, recreation activities, along with outdoor accommodations. To be eligible for admission to TLC facility, patients have to meet specific diagnosis criteria:

  1. 1) A life threatening illness with a life expectancy of 5 years or less; or
  2. 2) A terminal illness with a life expectancy of 6 months or less
